If a person was born or passed away outside of Wichita Falls City limits but within Wichita County, the certificate will be located at the Wichita County Clerk Office. A long form birth certificate is the most comprehensive certified birth record. It is a copy of the original birth certificate. It will also show a history of corrections that have been made to the birth record. A long form birth certificate are often required to obtain:
- U.S. Passport
- Dual citizenship
- Driver license
A short form or certified abstract of the birth record will only show current information for the individual’s name, date of birth, place of birth, sex, and name of parent(s). This form will not show a history of corrections. A short form birth certificate can be used for:
- School and/or sports registration
- Employment requirements
- Driver license

Welcome to the City of Wichita Falls - Vital Records Office Vital Statistics Division birth and death certificate online ordering system. Requests will be processed during business hours, Monday through Friday 8:00am to 5:00pm.
This is an official request from the Vital Statistics Division in Wichita Falls, Texas. Birth and Death Certificates are legal documents and will be treated as such. The information contained in this request is considered private. Please be sure to complete all fields within the order and upload the required information.
You will be notified by email when your order has been approved. At that time, your order is ready for pick-up, or your document will be mailed within one business day.

The Vital Records Office is responsible for filing births and deaths that occurred in the city limits of Wichita Falls. We are able to issue certified birth certificates for anyone that was born in Texas and certified death certificates for anyone that died in the city limits of Wichita Falls. For any deaths that occurred in surrounding towns of Wichita Falls, these records should be filed with the Wichita County Offices. Please call our office to verify before sending in a request.
All certificates issued from this office are certified copies of the original record on security paper that meets state requirements, which bears our Health Department Seal and the Texas State Health Department Seal within the paper. We do NOT use an embossed seal per the Texas Administrative Code Title 25 Part 1 Chapter 181 Subchapter B Rule 1813.28 –C- Security Features. Processing of Birth and Death Certificate Applications:
- You will receive an email notification when your application has successfully been submitted into the online ordering system.
- You may receive a second email notification or a phone call if our office has questions regarding your order or needs additional information to process your request.
- Once your application has been Approved, a confirmation email will be sent to notify you that your document is ready for pick-up, or will be mailed the following business day.
- If your application has been Denied, you will be notified by email. The payment amount for your order, less processing fees (Service, Credit Card, Vital Verify) will be refunded to the card you used.
* Warning: The penalty for knowingly making a false statement in this form can be 2-10 years in prison and a fine up to $10,000 (Health and Safety Code 195.003)
